PCN Staff Through the Havering South PCN Network, we have the below services available to our patients: - Physiotherapist - can diagnose and treat muscular and joint conditions. Advise on how to manage your condition. Refer you on to specialist service.
- Pharmacist - provides support to the team with medication and prescription queries.
- Health and Wellbeing Coach - support people to increase their ability to self-manage, motivation levels and commitment to change their lifestyle.
- Dietician - can diagnose and treat nutritional conditions. Help you make changes to prevent and support long-term conditions. Support you to maintain a healthy weight.
- Podiatrist: diagnose and treat a wide range of mobility and medical conditions of the feet and lower limbs.
- Care co-ordinator - help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time.
- Social Prescriber: connect patients to community-based support, including activities and services that meet practical, social, and emotional needs that affect their health and wellbeing. This includes connecting people to statutory services for example housing, financial and welfare advice.

Community Matron A community matron is a specialist trained nurse who can visit patients at home who have complex medical needs. Health Visitor A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
