Accessible Information & Reasonable Adjustments


Our commitment to accessible information


We are committed to making sure that everyone can access and understand the information we provide. If you have difficulty reading, seeing, hearing, or understanding information, we can offer it in a different format or communicate with you in a way that suits your needs.


This is in line with the NHS Accessible Information Standard, which ensures that patients with a disability, impairment, or sensory loss receive information they can access and understand.


We can provide information in formats such as:



  • Large print

  • Easy read

  • Braille

  • Email or text message

  • Telephone calls instead of written communication

  • Interpreters (including British Sign Language)


Reasonable adjustments


We understand that some patients need changes to how services are delivered to make sure they can access care fairly and safely. These are called reasonable adjustments.


Reasonable adjustments may include:



  • Longer appointment times

  • Quiet waiting areas

  • Support for communication during consultations

  • Step-free access or alternative consultation rooms

  • Support for anxiety, learning disabilities, or neurodivergent conditions

  • Allowing a carer, family member, or advocate to attend appointments


We will work with you to identify and agree any adjustments that will help you use our services.


Reasonable Adjustments Flag (RAF)


If you have ongoing needs that require adjustments, we can add a Reasonable Adjustments Flag (RAF) to your patient record.


The RAF is a national NHS system flag that helps healthcare staff quickly understand what support you need when you use NHS services. It ensures that your needs are recognised and consistently met across different healthcare settings.


The flag may include information such as:



  • Communication needs

  • Physical access requirements

  • Support needs related to disability or long-term conditions

  • Behavioural or sensory needs relevant to your care


You can request a Reasonable Adjustments Flag at any time by speaking to a member of our reception or clinical team.


How to request accessible information or adjustments


If you need information in a different format or would like to discuss reasonable adjustments, please contact the surgery. We will record your preferences and ensure they are shared with relevant staff involved in your care.


We encourage patients to tell us about their needs so we can provide safe, respectful, and effective care for everyone.
