PCN Staff
Through the Havering Liberty PCN Network, we have the below services available to our patients:
- Physiotherapist - can diagnose and treat muscular and joint conditions. Advise on how to manage your condition. Refer you on to specialist service.
- Pharmacist - provides support to the team with medication and prescription queries.
- Dietician - can diagnose and treat nutritional conditions. Help you make changes to prevent and support long-term conditions. Support you to maintain a healthy weight.
- Social Prescriber: connect patients to community-based support, including activities and services that meet practical, social, and emotional needs that affect their health and wellbeing. This includes connecting people to statutory services for example housing, financial and welfare advice.
District Nurses
The practice is supported by a wonderful team of District Nurses who can help support housebound patients.

Community Matron
A community matron is a specialist trained nurse who can visit patients at home who have complex medical needs.
The GP can refer you to the community matron but patients can also refer themselves by contacting the number or email below

Health Visitor
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
